MyChart for My Family

Stay on top of everyone’s appointments and check in on family members who need extra help, all from your MyChart account, through easy proxy access. A proxy is a person who has permission to view another person’s health information and take certain actions on that person’s behalf.
Can I view a family member's medical record in MyChart?
Yes, you can. MyChart proxy access allows you to access the medical records of your family members and others you care for, with their permission. You must be at least 18 years old to have proxy access to another person's medical record.
You might also want to grant a family member or friend access to your medical records when you need assistance managing your appointments and other medical needs. To give someone access to your medical record, grant access via a proxy invite through the Friends and Family Access page in MyChart.
The account holder will also need to choose an access class for their proxy and then verify the change. Adventist Health offers four options to choose from: Parent Accessing Child, Parent Accessing Teen, Full Access, and Clinical Access.
How do I get MyChart for my child, parent, or other person?
If you do not have a MyChart account of your own, you will need first to create your own account even if you are not a patient with Adventist Health. Then you can log in and set up a proxy account for your child or family member. Please speak with someone at your provider’s office via phone or during an appointment to get an activation code to begin this simple process.
Can my spouse and I share one MyChart account?
No, due to the sensitive nature of medical information, each person must have their own MyChart account.
How is proxy access to MyChart determined by a patient's age?
- Children 12 and younger: Parents/guardians need a MyChart account of their own, then they can add the child to their account and connect to their records. Only parents or legal guardians may request access to their child’s account. You can get proxy access online, or by visiting, or calling their provider’s office. In California, parents will receive information about the change in proxy settings within 30 days of the minor’s 12th birthday.
- Children 13 and older: In Washington, Children 13 and older must have their own MyChart account. Parents will receive information about the change in proxy settings within 30 days of the minor’s 13th birthday.
- Under age 15: Only parents or legal guardians may request access to the accounts of children under the age of 15. You can get proxy access online or by visiting or calling their provider’s office.
- Age 15 and older: For children 15 and older, the child must fill out and submit a release of information form to grant their parent/guardian access. In Oregon and Hawaii, parents will receive information about the change in proxy settings within 30 days of the minor’s birthday.
- Age 18: On your child’s 18th birthday, their MyChart automatically becomes an adult account. They will have to follow instructions for adult proxy access in order to restore your access to their MyChart, and a release of information will need to be filled out.
- Adults: For you to gain parent or caregiver proxy access to another person’s medical information, the patient must fill out and submit a release of information form to grant your access.
Once I have been granted access, how do I view other people’s MyChart?
Once you have proxy access, you can view those additional health records from your own MyChart account. Just click the tab on the top right of your account to switch between accounts.
