Beginning August 30, 2026, MyChart records from visits and procedures at Adventist Health Portland will be available in the new Adventist Health MyChart. This is a part of Adventist Health's new and improved patient portal, which is designed to give you easier, more secure access to your health information. Whether you’re managing appointments, viewing lab results, or messaging your care team, MyChart is your trusted companion for staying informed and connected. Visits at OHSU or Hillsboro Medical Center will remain in your OHSU Health MyChart account, as will any MyChart messages sent to any providers, including Adventist Health Portland before August 28.

Continue staying connected to your care with MyChart 

We’re here to make sure it’s easy for you to stay connected to your care at Adventist Health.

  • You can add Adventist Health MyChart via your MyChart app starting on Aug. 30, 2026. You will be able to log in the first time with your current OHSU Health MyChart username and password
  • You’ll continue to find OHSU Health information in your OHSU Health MyChart account
  • Your information for care received from Adventist Health prior to August 30 will transition to your Adventist Health MyChart account
  • Proxy access will transfer automatically

Helpful information about the MyChart transition 

If you’re using MyChart today, you know how it puts your health journey at your fingertips. The information below can help you learn more about the transition process and what you can expect.

What do you need to do?

Your OHSU Health MyChart login information will be securely transferred over to your Adventist Health MyChart account, so you should not need a new username or password.  However, these are separate accounts, so if you change your password or username in the future in one of the accounts, it will not automatically apply to both.

Will I still have access to my records?

Yes, your visits at OHSU and Hillsboro Medical Center will continue to be available in your OHSU Health MyChart account. All of your visits from Adventist Health Portland prior to Aug. 30 will be transferred over and available in your Adventist Health MyChart account. This does not include MyChart messages.

How do I link my MyChart accounts?

Starting on August 30, 2026, after signing in to MyChart:

  1. Select Menu
  2. Choose Link My Accounts
  3. Follow the prompts to link your Adventist Health and OHSU MyChart accounts

Once connected, eligible information from both health systems can be viewed together within MyChart. 

What about proxy access?

Proxy access for family members and caregivers will transfer automatically when selecting Adventist Health as your healthcare provider. No additional action is needed.

Haven’t created your account yet? 

Go to MyChart and select “Sign Up Now.” You will need your access code and medical record number. This can be found on the last page of the after-visit summary you received at the end of your appointment. If you lost your access code or do not have one, please contact the MyChart help desk at 833-AHCHART or 833-242-4278

Accessing MyChart for a child or loved one

Sharing medical information can be a convenient way for parents and caregivers to help their loved ones. To protect patient information, you must take simple steps to share access with others.

How do I get MyChart for my child, parent, or other person?

If you do not have a MyChart account of your own, you will need first to create your own account even if you are not a patient with Adventist Health. Then you can log in and set up a proxy account for your child or family member. Please speak with someone at your provider’s office via phone or during an appointment to get an activation code to begin this simple process. 

The age of the person you accessing MyChart for informs how you can gain access. 

  • Children 12 and younger:  Parents/guardians need a MyChart account of their own, then they can add the child to their account and connect to their records. Only parents or legal guardians may request access to their child’s account. You can get proxy access online, or by visiting, or calling their provider’s office. 
  • Children 13 and older: Children 13 and older must have their own MyChart account. 
  • Under age 15: Only parents or legal guardians may request access to the accounts of children under the age of 15. You can get proxy access online or by visiting or calling their provider’s office. 
  • Age 15 and older: For children 15 and older, the child must fill out and submit a release of information form to grant their parent/guardian access. 
  • Age 18: On your child’s 18th birthday, their MyChart automatically becomes an adult account. They will have to follow instructions for adult proxy access in order to restore your access to their MyChart, and a release of information will need to be filled out. 
  • Adults: For you to gain parent or caregiver proxy access to another person’s medical information, the patient must fill out and submit a release of information form to grant your access.

Patients 15 and older must fill out and submit a signed release of information form. You may request a form at the patient's next appointment.

Once I have been granted access, how do I view other people’s MyChart?

Once you have proxy access, you can view those additional health records from your own MyChart account. Just click the tab on the top right of your account to switch between accounts.
