No matter what kind of health issues impact your life, you can still get better and live well! Our Live Well program helps you do just that with a holistic approach that cares for the whole you: body, mind and spirit. We'll partner with you to set goals and create a personalized care plan. We'll assemble your own team of care providers, educators, and coaches to provide the support you need to better manage your physical, mental, and emotional condition.

Your path to living well
Live Well involves a series of steps and services to get you on track to healthier, happier living. Learn more below.
Your health assessment
Your first step with Live Well is to meet with a nurse practitioner and for a physical and behavioral health assessment. The nurse will review all aspects of your health, medical history, diet, lifestyle, and emotional/psychological condition. The information gathered from your health assessment will be used to develop your personalized care plan.
Your personalized care plan
With the information gathered from your assessment, we’ll partner with you to set your health goals, then put together a care plan to help you accomplish them. Your plan will include the team, tools, and support you need to best manage your condition and create a healthier, happier life for yourself.
Your care management
A care manager will oversee your program, help you with scheduling appointments or referrals, and answer any of your questions. We know it can be overwhelming to understand all the various healthcare actions and interactions that support a healthy lifestyle. Your care manager will help you make sense of it all.
Your care team and services
Based on your care plan, we’ll customize a team of healthcare professionals who will provide you with the services, guidance and support you need to reach your goals. You may see several practitioners or just one. Depending on your needs, your team and services might include:
- A health coach to provide fitness evaluations and a supervised exercise regimen customized to your needs, age, strength, and other factors. It’s like having your own personal trainer.
- Nutritionists or dieticians to make sure your diet is helping, not hurting, your progress to your health goals.
- Psychiatrists, psychologists, or licensed clinical social workers (LCSWs) to provide support and treatment for depression, anxiety, and other mood disorders.
- Diabetes educators to help you learn more about diabetes management and lifestyle improvements that can prevent diabetes-related health crises.
- A chiropractor to provide relief from back pain, headaches, arthritis, and more.
- Sleep specialists to diagnose and treat any sleep disorders so you can get the sleep you need for a healthier, more productive quality of life.
- A substance use counselor to help you find freedom from addiction.
- A prosthetist/orthotist (CPO) to provide devices like special braces or insoles if you have mobility issues.
- A pain management specialist to provide the right diagnosis and treatment if you suffer from chronic pain.
- Support groups to learn more, connect with and gain support from others who are dealing with the same condition(s) you are.
Tracking your progress and adjusting your care
As you continue with Live Well, your nurse practitioner will collaborate with your team to do regular assessments of your progress and may adjust your care plan to include additional services, or to focus more on a particular area. Your team will also provide you with feedback and plenty of encouragement so you can stay on track to your goals and maintain a healthier, happier you!

Should I still see my primary care provider if I join Live Well?
Yes. In fact, your primary care provider might refer you to Live Well. Your Adventist Health provider will also be able to read your Live Well progress reports in your electronic medical record. All your information will stay in one place so we can provide you with the best care.
Can I do only one part of the program?
Yes, but there are huge benefits to following your entire recommended care plan, especially if you have multiple health issues. That’s because all components of your plan work holistically together for optimal health and wellness. However, you can see nutrition and behavioral health providers without accessing the Live Well program.
Can I go to physical therapy and Live Well at the same time?
Yes. Typically, it is preferred that your physical therapy is completed prior to beginning exercising through Live Well, but that is not necessary.
What insurance plans does Live Well accept?
The Live Well Program welcomes all insurances, including Medi-Cal, Medicare, and Partnership HealthPlan of California. Our staff is here to help you with your questions about deductibles and copays.
