Billing
Please reach out to us for help with paying your bill or if you have questions about your bill. Call 800-593-1551.
Insurance
Adventist Health welcomes Medi-Cal, Medicare, and most insurance plans. We also offer a discount program for the uninsured. If your current plan does not cover care at one of our facilities, ask your benefits manager or human resources department to add or consider switching to a plan that does.
Medical records and patient portal
Request your medical records online or complete a paper form in English or Spanish. For paper forms, please submit a scanned copy of your completed paper authorization form by email to MedRecords@AH.org.
You may also utilize the patient portal for access to your personal health information.
Advance Directive
Know your rights to make healthcare decisions, including when you can’t speak for yourself. Federal law requires us to provide information about Advance Directives, which we hope will help you increase control over your medical treatment. Ask your care provider about an Advance Directive for you or your loved ones.

Do you know of any nurses who provide exceptional care at our hospitals? If so, please consider nominating them for the Daisy Awards.

Resolving concerns
We are committed to providing excellent care. If you feel we aren’t delivering on this commitment, please speak to any staff member. A unit or department supervisor can also assist you.
If you still have concerns, please contact or talk with the director of the department about your concern, and how we can best resolve your issue. You may also send us an email at patientrelations@ah.org.
