Our diabetes care and education services are designed to help prevent complications and promote successful self-management. In collaboration with you and your primary care physician, we can customize a personalized care plan of the services you need based on your condition and other factors. Diabetes and prediabetes management
Our diabetes management sessions, led by registered nurses and dietitians, offer practical guidance on nutrition, medications, blood glucose control, hygiene, sick day management, stress reduction, exercise, and community resources. The program includes an individual pre-assessment and follow-up consultation, designed to complement your doctor’s care. A referral from your healthcare provider is required to participate.
Gestational diabetes management
This program is offered for women diagnosed with diabetes during pregnancy and is taught by certified diabetes educators. Topics covered include nutrition, exercise, stress management, breastfeeding, glucose monitoring, medications, the impact of diabetes on your baby and more. A physician's referral is required.
Continuous glucose monitoring
Continuous Glucose Monitoring (CGM) therapy provides real-time tracking of glucose levels around the clock, helping individuals manage their glycemic goals and address frequent hypoglycemia. A physician’s order and referral are required to access this therapy.
Individual nutrition counseling
With a referral from your physician, we offer personalized, one-on-one nutrition sessions to help you improve your diet and make healthier choices that can minimize your risk of diabetes-related and other chronic health conditions. Our outpatient dietitians collaborate with you to create realistic, sustainable goals tailored to your specific needs. We offer nutrition counseling for:
- Cancer
- Diabetes management
- General healthy eating
- Heart disease and cardiac rehab
- Kidney disease
- Weight management (loss or gain)
- Meal planning
- Weight loss surgery support
