Diabetes is a life-changing diagnosis, but you don’t have to let it control you. You can take control of your health through our diabetes management services. Our certified diabetes educators and dietitians will give you the skills and knowledge to understand diabetes, monitor and manage your blood sugar levels, and make important lifestyle, and dietary changes to achieve a healthier life with fewer complications.
Our services and support
Our certified diabetes educators, nurses, and registered dietitians will partner with you and your physician to develop a personalized plan that meets your specific needs and helps you stay on track to better quality of life.
Diabetes management program
Our comprehensive diabetes management program involves in-office visits, a series of meetings, and classes for a period of several months, or longer. Depending on your needs, this may include:
- One assessment visit
- Small-group program
- Four bi-weekly classes
- A six-week follow-up visit
- Ongoing treatment and support as needed
In these meetings and classes, we will cover a variety of topics based on your personalized plan, including:
- Monitoring and controlling your blood sugar
- Continuous glucose monitoring system set-up and training
- Warning signs of diabetes-related complications
- Diabetes-related care services such as chronic wound care
- What to eat and what to avoid for better diabetes management and overall health
- Weight management and fitness regimens to control diabetes
- Realistic and sustainable goals
- Reducing risks and managing complications
- Other lifestyle changes you can make
Monthly diabetes support group
Studies have shown that support from peers can help improve clinical and behavioral outcomes for patients with diabetes. That’s why we offer a safe space for people living with diabetes to share stories, tips, and serve as a supportive community to help you thrive. Support group meetings are held once a month, every Monday at the hospital.

Gestational diabetes support
Some women may develop diabetes during pregnancy. This condition is called gestational diabetes, and it affects up to 9% of pregnancies in the United States every year. Our diabetes education team is especially trained and experienced in helping you navigate gestational diabetes to keep you and baby healthy.
With our team’s support, you’ll learn how to manage your condition through lifestyle changes and monitor your blood sugar to avoid complications.
