Improving access to care for the most vulnerable
Navigating the healthcare system can be complicated, even more so for the unhoused, and our most vulnerable neighbors. Our COMPASS care management services ensure that no one gets left behind and can get the help and care they need.
Our areas of focus
COMPASS provides short-term medical care management at our hospital free of charge for individuals and families struggling with homelessness, low incomes, disabilities, and other challenges. Our focus is on wellness and prevention to help avoid crisis situations that lead to emergency room visits or hospitalization. We can help with:

Care beyond our walls
For some individuals, getting to our hospital may be difficult. So we’re dedicated to delivering assistance and care wherever you're at through these additional services outside our hospital.
Street Medicine Program
The Street Medicine Program in Mendocino County is a collaborative effort between Adventist Health's COMPASS street medicine team, several local community service providers, and the Building Bridges Shelter, all working together to bring urgent care to the unhoused population.
We value this community-embedded healthcare delivery model that provides care far beyond our hospital walls and is uniquely tailored to the needs of the rural community.

Weekly walk-in clinics
We also offer weekly walk-in health clinics at various locations in the community. During these clinics, we offer wound care, health education, help with medication, and connecting you with other resources. For locations and times, please contact us.
