Home Care

Working in conjunction with you and your primary care provider, we develop a personalized care plan that includes the services and types of care providers you require. Your visits will be scheduled according to your needs, while your progress will be monitored and reported back to your primary care provider to ensure seamless continuity of care. 

Depending on your personalized plan, the team providing care may include: 

• Registered nurses 
• Home Health aides 
• Medical social workers 
• Physical, occupational and speech therapists

In addition to serving patients who are elderly, disabled or have special needs, home care services are suitable for those who are incapacitated due to a medical condition, such as: 

• Acute injury or illness 
• Recent surgery Infectious diseases 
• ALS
• Cancer
• Lung or pulmonary diseases 
• Congestive heart failure 
• Diabetes 
• Parkinson’s disease

Depending on your care plan, your home care might include any of the following: 

• Monitoring your vitals 
• Medication assistance 
• Wound care and management 
• Home infusion/intravenous medication 
• Palliative care 
• Disease management 
• Physical therapy 
• Speech therapy 
• Occupational therapy 
• Social services

Home health often costs less than traditional hospital or nursing care. To be eligible for home health, the patient must: 

• Be home bound, or unable to leave their home without some degree of difficulty. 
• Require skilled care. 
• Have a referral or prescription for home care from their physician or qualifying healthcare facility.

Most private insurance companies cover home health services prescribed by a physician. Medicare, Medicaid, worker’s compensation, and Veterans Services also offer coverage for home health. For copay costs and any services not covered by insurance, Adventist Health also accepts private payment.
