Secure, online access to your health records in MyChart
Accessing MyChart
Using your computer or the MyChart app, you can go online to access your health records through a secure, encrypted connection from anywhere. You can use OHSU Health MyChart to:
MyChart Virtual Visits
When you schedule a virtual visit, you'll start your visit through your OHSU Health MyChart patient portal. Here are the steps to take so you can be ready to connect with your provider.
Up to seven days before your visit
- Download the free Zoom app on your phone, tablet or computer.
- Visit OHSU's virtual visit preparation page for instructions and how-to videos.
- Test your sound, microphone, and video at Zoom.us/Test.
- Complete your PreCheck-in (required) to update your medications, answer questions about your health, and save time during your virtual visit.
- Log in to MyChart.
- Go to visits.
- Tap your appointment.
- Tap precheck-In.
At least 15 minutes before visit
Check in electronically at least 15 minutes before your visit. Many people find they need more time, so please give yourself plenty.
- Log into MyChart.
- Complete your precheck-in if you haven’t already (You can do this up to seven days before your visit. See above.)
Join your visit
- Fifteen minutes prior to your appointment, tap the “Join video visit” icon (if you haven't already downloaded Zoom, you will be prompted to do so).
- You will automatically enter a virtual waiting room. Your provider will join you there.

Additional information
A provider must be licensed in the state where a patient is physically located at the time of visit. If you think you’ll be outside of Oregon at the time of your visit, please contact your clinic.
Please do not join your virtual visit while operating a motorized vehicle. Your provider may ask you to reschedule if you do so.

Haven’t created your account yet?
Go to MyChart and select “Sign Up Now.” You will need your access code and medical record number. This can be found on the last page of the after-visit summary you received at the end of your appointment. If you lost your access code or do not have one, please contact the MyChart help desk at 503-494-5252.
Accessing MyChart for a child or loved one
Sharing medical information can be a convenient way for parents and caregivers to help their loved ones. To protect patient information, you must take simple steps to share access with others.
How do I get MyChart for my child, parent, or other person?
If you do not have a MyChart account of your own, you will need first to create your own account even if you are not a patient with Adventist Health Portland. Then you can log in and set up a proxy account for your child or family member. Please speak with someone at your provider’s office via phone or during an appointment to get an activation code to begin this simple process.
The age of the person you accessing MyChart for informs how you can gain access.
- Children 12 and younger: Parents/guardians need a MyChart account of their own, then they can add the child to their account and connect to their records. Only parents or legal guardians may request access to their child’s account. You can get proxy access online, or by visiting, or calling their provider’s office.
- Children 13 and older: Children 13 and older must have their own MyChart account.
- Under age 15: Only parents or legal guardians may request access to the accounts of children under the age of 15. You can get proxy access online or by visiting or calling their provider’s office.
- Age 15 and older: For children 15 and older, the child must fill out and submit a release of information form to grant their parent/guardian access.
- Age 18: On your child’s 18th birthday, their MyChart automatically becomes an adult account. They will have to follow instructions for adult proxy access in order to restore your access to their MyChart, and a release of information will need to be filled out.
- Adults: For you to gain parent or caregiver proxy access to another person’s medical information, the patient must fill out and submit a release of information form to grant your access.
Submitting a signed release of information form
Patients 15 and older must fill out and submit a signed release of information form. Please print, fill out, and sign the form. You may also request a form at the patient's next appointment.
Download the Release of Information Form
To submit it:
- Via email: Scan or photograph the form as well as the patient’s government-issued ID. Save them as PDF or JPG files. Email all the files to AMCPMyChartSupport@AH.org.
- In person: Bring the completed form to the patient’s next clinic appointment.

Once I have been granted access, how do I view other people’s MyChart?
Once you have proxy access, you can view those additional health records from your own MyChart account. Just click the tab on the top right of your account to switch between accounts.
