Resolving Concerns

Your experience matters to us, and we welcome your feedback. If you want us to know something about your experience, we recommend the following steps so we can address and resolve your concerns.

  1. Speak up right away. The best time to make sure we meet your expectations is in the moment. Please feel free to tell us what we can do better. If you're having an issue with a particular staff member, please ask for a supervisor or manager and discuss your concerns.
  2. Ask for more help. If your concerns are not resolved by talking with your care team, fill out the online patient feedback form so we can get your information directed to the correct department or clinic. Be sure to check the option to be personally contacted if you want a reply. Patient Feedback Form
  3. Reach out to the Adventist Health system. If you're not satisfied with the response you receive from Adventist Health Portland, you can file a complaint with the Adventist Health system by calling 888-366-3833.
  4. Report safety concerns to the Joint Commission. If your concerns involve a safety issue and cannot be resolved through the Adventist Health organization, you can contact the national health care accreditation program, The Joint Commission, in three ways:
    1. At JointCommission.org, using the “Report a Safety Concern” link.
    2. By fax: 630-792-5636
    3. By mail: Office of Quality and Patient Safety, The Joint Commission, 1 Renaissance Boulevard, Oakbrook Terrace, IL 60181
