Viewing your medical records
Access to your medical records is important! It’s a great way to actively participate in your healthcare and understand conditions and treatments. When you understand your medical history and test results, you’re best equipped to ask questions and stay on track with screenings, vaccinations and preventive care.
View your records on your patient portal
With Adventist Health’s patient portal, MyAdventistHealth, you have a direct link to access your own personal electronic health records immediately. You can log in to MyAdventistHealth now to do the following:
Please note that not all medical records are viewable and/or retrievable through the patient portal. For records that are not available through the patient portal, you can submit a request for them.
Requesting medical records
Patients and providers may request medical records using the methods below. We’ve made it easy for you and will respond promptly. We encourage online requests
Patient online request
Request your records be sent to you by completing our simple online request form.
Provider online request
Providers may request medical records using our online request process. Our paperless process does not require a login. Instead, we use email verification for enhanced security, saving the time of faxing, scanning and printing.
Patient paper request
If you prefer a paper authorization form, please download and print the appropriate form below and submit a scanned copy by email to MedRecords@ah.org. You may also submit your completed form by fax or mail. See below for the address and fax number for our medical records department.
Patient in-person request
We encourage you to submit your request online or by paper form. If you wish to visit the Medical Records department in person, please call ahead.

Requesting other records and information
Copy Service and Subpoena
A separate subpoena is required for each department that documents are requested from (i.e., images, specimen, or billing). For follow-up on a request, contact the department in which the subpoena was served.
Itemized Hospital Bills
Please contact our Patient Financial Assistance department. If you have a question about a physician’s office bill, contact your physician’s billing office directly.
Laboratory and Pathology
Results may be obtained from the Medical Records Department. Test samples, slides and specimens may be obtained from the Laboratory/Pathology Department.
Risk Management
All other types of requests will be sent to the facility Risk Management department to determine the appropriate resource to respond to the request (i.e., subpoena or search warrant.)
Timing and fees
Requests are typically fulfilled within 14 business days. There are no fees associated with requests sent directly to a medical provider. Fees may be associated with requests that are sent to you or to someone who is not a medical provider. If there are any fees associated with your request, you will be invoiced by our business partner, Datavant. All fees are in accordance with state and federal laws.
