Cardiac rehab is an important part of the treatment of a range of heart conditions. If you have one of the following diagnoses, you may be eligible to participate:

- Cardiac surgery, such as coronary bypass, or valve surgery
- Coronary artery angioplasty or stents
- Heart attack
- Heart failure
- Heart transplantation

How to start cardiac rehabilitation
A referral is required to begin cardiac rehabilitation services. Talk with your cardiology or primary care provider to see if it is right for you.
Once you’re referred, the team of cardiac rehabilitation specialists will start working with you to develop a customized rehab plan. Your program will include:
- Assessment of your personal risk factors for heart and blood vessel disease.
- Beginning and maintaining a personalized exercise plan that works for you.
- Psychological/stress assessment and counseling.
- Education and support to make healthy lifestyle changes.
Cardiac rehab is often covered by health insurance. We recommend you check your plan to determine copays or other requirements. Contact us to inquire about programs for patients who do not have insurance.
