Billing and insurance

Our team is available at 541-296-7504 Monday through Friday, 7 a.m. – 5 p.m. Feel free to leave a message after hours.

Medical records

To obtain your medical records, submit a request to our health information management department. Call us at 541-296-7294 with any questions.

Some records will be available automatically in your online electronic record portal, MyChart. Your clinic provider can give you access next time you visit. If you bring photo ID, our health information management staff can also sign you up for this free service. 

Patient portal

With MyChart patient portal, you have a direct link to access your own personal health records immediately. Log in to MyChart now to do the following:

  • View your test results such as lab results, radiology reports, and pathology reports.
  • Download or reprint education documents and discharge instructions.
  • View or download summaries of visits and complete health summaries.
  • Access your health records, such as immunizations, allergies, and medications.

Please note that not all medical records are viewable and/or retrievable through the patient portal. For records that are not available through the patient portal, see medical records.  

Resolving concerns

If you want us to know something about your experience, we recommend the following steps so we can address and resolve your concerns.

  1. Speak up right away
    The best time to make sure we meet your expectations is in the moment. Please feel free to tell us what we can do better. If you’re having an issue with a particular staff member, please ask for a supervisor or manager and discuss your concerns. 
  2. Ask for more help
    If your concerns are not resolved by talking with your care team, contact Quality Resources at 541-296-7285 or AHCGQualityResources@ah.org.
  3. Reach out to the Adventist Health system
    If you’re not satisfied with the response you receive from Adventist Health Columbia Gorge, you can file a complaint with the Adventist Health system by calling 888-366-3833.
The Joint Commission

If your concerns involve a safety issue and cannot be resolved through our organization, you can contact the national healthcare accreditation program, The Joint Commission.

Visit JointCommission.org and click on Contact Us in the navigation. Then choose “Submit a Patient Safety Complaint” under the “What Can We Help You With?” section.
