Did you know Adventist Health has a dedicated Transitional Care Management team to support patients being discharged from the hospital?
Within 48 hours after leaving the hospital, a member of our Transitional Care Management team will contact patients to help coordinate follow-up care. They can assist with scheduling provider appointments, reviewing medications, developing personalized care plans, connecting patients with community resources, and answering questions to support a smooth recovery and reduce the risk of needing to return to the hospital. "Seeing firsthand the difference this work makes in the lives of our patients is both humbling and incredibly rewarding," says Trista Campos, Administrative Director of Transitional Care Management. "At Adventist Health, we understand the unique needs of our communities, and it is an honor to provide this important aspect of patient centered care. "
Transitional Care Management services are available to patients who are within the Value Based Population (AHCN, AHP, ACO MSSP) discharged from Adventist Health hospitals in Hanford, Tulare, Selma, Reedley, Glendale, White Memorial, Montebello, and Simi Valley.
