Adventist Health Home Health and Hospice services provides the support your family needs. From in-home care to getting your loved one through rehabilitation, or coping with life-limiting illnesses, we are here for you. 

Comfort and care 

Coping with a life-limiting illness can be frightening and even overwhelming. Hospice care can help ease the burden. Adventist Health Hospice services can provide: 

  • Personal care assistance
  • Help minimizing pain and symptoms while maximizing comfort, quality of life, and mental well-being
  • Mental and emotional support for patients and families
  • Spiritual support
  • Nutritional counseling
  • Bereavement and grief counseling
  • In-residence therapy
  • Assistance with accessing community resources and services
  • Help with setting up advanced directives
  • Respite care for primary caregivers
  • Funeral and memorial planning

Healing at home 

We offer skilled, in-home health care from providers committed to the safety, comfort, and independence of our patients. Our Home Care team works closely with physicians to provide exceptional care that is personalized for each patient. Nurses are available 24 hours a day, 7 days a week, for scheduled or on-call visitations. 

In-home education and training services

Our caring and skilled providers offer education and training in a wide range of home care topics, including: 

  • Activities of daily living, personal care, bathing, and dressing
  • IV home therapy, including how to administer and manage IV infusion and prevent infections
  • Proper nutrition, feedings, and ways to prevent aspiration  
  • Information and self-care techniques for managing acute and/or chronic conditions, when to seek medical attention, and when and how to properly take medications
  • Diabetes management, such as proper diet, medications, checking/monitoring sugar levels in blood, and how to choose user-friendly equipment, such as a glucose meter
  • Monitoring hydration, detecting early signs of dehydration, and preventing infections
  • Ostomy and wound care, including how to care for stoma or wounds, prevent infections, and other self-care techniques
  • Home rehab exercises, gait, and muscle strengthening, and home safety
  • How to access help for social, emotional and financial needs, and connecting with community resources
